IOWA DERMATOLOGY'S FINANCIAL POLICY We are
dedicated to providing the best possible dermatologic care for you, and we want
you to completely understand our financial policies. 1. Payment is due at
the time of service unless arrangements have been made in advance by you or
with your insurance carrier. For your convenience, we do accept Visa and
MasterCard credit cards for payments.
2. Your insurance policy is
basically a contract between you and your insurance company. As a service to
you, we will file your insurance claim if you assign the benefits to the
doctorin other words, if you agree to have your insurance company pay the
doctor directly. If your insurance company does not pay the practice within a
reasonable period, you are ultimately still responsible for payment in full. If
we later receive a check from your insurer, we will refund any overpayment to
you.
3. We have made prior arrangements with many insurance companies
and other health plans to accept an assignment of benefits. Iowa Dermatology
will submit claims to your insurance carrier, but you may be required to pay a
co-payment, co-insurance or a deductible payment at the time of your visit,
depending on your insurance plan.
4. If you are insured by a plan that
we do not have a prior arrangement with, we may prepare and send the insurance
claim in for you on an unassigned basis for your convenience. This means the
insurer will send the payment directly to you. Therefore, our charges for your
care are due from you at the time of service.
5. Not all insurance
plans cover all services. In the event your insurance plan determines a service
to be not covered, you will be responsible for the complete charge
for that service. Payment in full is due within two weeks upon receipt of a
statement from our office.
6. We will bill your insurance company for
all services provided in the hospital. You are responsible for any balance due.
7. Failure to provide 24 hours notice of a cancelled appointment may
result in financial penalty being imposed on future missed appointments. A
deposit will be required before future appointments are scheduled. Missed
appointments will result in a debit on the deposit.
8. Failure to meet
your financial obligations within a reasonable period of time will result in
your termination as a patient and your account will be forwarded on to a
certified collection agency.
9. The practices financial policy is
not flexible and by attending an appointment with the doctor, you agree to be
bound by its terms. |
